The honest landscape

How carriers actually see high cholesterol

On final expense, high cholesterol is about as close to a non-issue as health conditions get. Royal Neighbors’ risk-assessment grid marks “Cholesterol Treatment” as available at every class, Preferred included — on that grid, cholesterol treatment and hypertension are the only heart-related items that can reach Preferred. United Home Life’s 2026 guide places cholesterol treatment at Premier, its best whole life plan, and at Term Premier. And in Transamerica’s own underwriting case study, a client taking Lipitor alongside other medications was placed by her diabetes, not her statin.

Where cholesterol can matter is through what else your prescriptions say. Many drugs have more than one use, and Aetna’s drug guide tells agents the application must show the condition a multi-use medication is prescribed for. American Home Life’s guide shows why: Caduet, a combined blood-pressure and cholesterol pill, is rated Level for high cholesterol or hypertension but Graded when it’s treating angina diagnosed or treated in the last 24 months. If your statin came with a stent, a heart attack, or chest pain, those histories set your tier.

Fully underwritten term is where the numbers count, through total cholesterol and the cholesterol-to-HDL ratio. Royal Neighbors’ Jet guide requires no treatment history, a total of 220 or less, and a ratio of 5.0 or less for Super Preferred, but sets Preferred at 240 and 6.0 with no treatment requirement; Foresters publishes limits by age band, up to 300 for Standard Plus. And if other conditions ever close simplified issue, guaranteed-issue whole life asks no health questions — Mutual of Omaha’s runs to age 85, Corebridge’s and Americo’s to 80, with a graded period for natural-cause death in the first two years (three at Americo) and full accidental coverage from day one.

Your realistic paths

The three doors, honestly labeled

Essentially always, when cholesterol is the issue

Day-one level coverage

Royal Neighbors and United Home Life place cholesterol treatment at their best classes, and we found no final expense guide that rates treated cholesterol below its top tier.

Not driven by cholesterol

Graded coverage

Cholesterol alone doesn’t route anyone to a graded plan. If you’re there, it’s a heart history traveling with it — a stent, angina, or a heart attack — and those have their own clocks.

Not needed for this (ages 45–85 if it ever is)

Guaranteed acceptance

No health questions. Mutual of Omaha to 85, Corebridge and Americo to 80. Graded period for natural causes; accidents covered in full from day one.

What moves the needle

Climbing the ladder

Know what each prescription is for

Multi-use medications are rated by the condition they treat. A combination pill for cholesterol and blood pressure is a non-event; the same pill for recent angina is not.

Stable treatment for 12 months

Fidelity Life’s InstaBrain term accepts elevated cholesterol at every class when it was diagnosed after 18, is monitored by a doctor, and has had no new medications or dosage changes in the past 12 months.

The ratio, not just the total

Fully underwritten classes look at your cholesterol-to-HDL ratio as well as the total — Royal Neighbors’ Jet guide and Foresters both publish ratio limits by class — so improving HDL can matter as much as lowering the total.

Inside the application

What carriers actually ask

  • Royal Neighbors’ Ensured Legacy grid: “Cholesterol Treatment” — available at Preferred, Standard, graded, and guaranteed issue.
  • Aetna’s drug guide, to agents: when a medication can be prescribed for more than one condition, “you must provide the condition the medication is prescribed for.”
  • American Home Life’s drug guide rates Caduet (amlodipine and atorvastatin) Level for high cholesterol or hypertension — but Graded when it’s for angina diagnosed or treated within 24 months.
  • Royal Neighbors’ Jet guide, Super Preferred: “No treatment hx; ≤220” total cholesterol and a ratio of 5.0 or less; Standard: 241–299, with a ratio of 6.1–8.5.

High cholesterol & life insurance — FAQ

Will taking a statin raise my life insurance rate?

Not on final expense — Royal Neighbors and United Home Life place cholesterol treatment at their best classes, and we found no final expense guide that rates it lower. On fully underwritten term, the statin itself isn’t the issue; your total cholesterol and cholesterol-to-HDL ratio are, and treatment that brings them into range helps. Royal Neighbors’ top Super Preferred class is the rare exception that requires no treatment history.

Do I need a blood test for life insurance with high cholesterol?

Not for final expense or other simplified-issue coverage — those use health questions plus prescription and medical-data checks, not labs. Fully underwritten term typically includes a paramedical exam with blood work for larger amounts, and that’s where published cholesterol limits apply.

My cholesterol medicine is also for my heart — does that change things?

Yes — the condition drives the rating, not the pill. Aetna requires agents to list the condition a multi-use drug is prescribed for, and American Home Life rates a combination cholesterol-and-blood-pressure pill Level for cholesterol but Graded when it’s treating recent angina. If you’ve had a stent, heart attack, or chest pain, those clocks decide your tier.

What cholesterol numbers do fully underwritten carriers look for?

They publish limits by class. Royal Neighbors’ Jet guide: Super Preferred 220 or less with a ratio of 5.0 and no treatment history; Preferred 240 and 6.0; Standard 241–299 and up to 8.5. Foresters’ 2026 guide runs from 230–250 (by age band) for Preferred Plus up to 300 for Standard Plus. Every carrier draws these lines a little differently, which is why we compare.
